7.2.3 Learning

No matter the failure, there is always something to learn from it. A CTO who has never experienced failure hasn’t yet led. Recording and documenting the issues is an important data-gathering exercise that will let you analyze the issues holistically. A list of potential issues you could document follows:

Does a problem keep coming up repeatedly?—This suggests the processes you are putting in place to prevent the problem aren’t working.

Are problems isolated to a handful of clients?—For organizations that are very client centric, it could be something unique about a given client that makes it hard to replicate with others.
